I. Application by Corporation
1. The applicant shall fill in the APPLICATION FORM FOR CHECK & ACCEPTANCE FOR ENVIRONMENT PROTECTIONS UPON COMPLETION OF CONSTRUCTIONAL PROJECTS or the APPLICATION REPORT FOR CHECK & ACCEPTANCE FOR ENVIRONMENT PROTECTIONS UPON COMPLETION OF CONSTRUCTIONAL PROJECTS or the REGISTRATION CARD OF APPLICATION FOR CHECK & ACCEPTANCE FOR ENVIRONMENT PROTECTIONS UPON COMPLETION OF CONSTRUCTIONAL PROJECTS in four copies (the Form for Check & Acceptance on Projects for implementing control programs within specified time limit), and make the authorization for surveys.
2. Submission of needed information documents:
1) Xeroxed copy of the written approval on the report for environmental impacts by constructional projects (or the statement or registration form); the Xeroxed copy of the written approval on the projects for implementing control program within specified time limit for the check and acceptance of the project involved;
2) Xeroxed copies of the designing plans and the qualification certificate of the designing institution;
3) Adjustment and testing report.
II. Procedures for surveys on check and acceptance:
1. The corporation makes the authorization for check and acceptance survey;
2. Plans for undertaking the surveys are to be worked out;
3. Field surveys are to be made on the spot;
4. A SURVEY REPORT ON CHECK & ACCEPTANCE FOR ENVIRONMENT PROTECTIONS UPON COMPLETION OF CONSTRUCTIONAL PROJECTS is to be supplied;
5. For some significant projects or projects involving special requirements, specialists are to be invited to do the field survey and to form a joint remark of the specialist team.
III. Comments and Remarks on Check and Acceptance:
With the authorization of the municipal Environment Protection Bureau, Zhuhai Environment Protection Monitoring & Surveying Station shall work out a general comment on the check & acceptance of the project and issue the LICENSE FOR DISCHARGING CONTAMINANTS upon completion of the survey mission and on the basis of the check & acceptance information documents for environment protection facilities supplied by the applicant corporation as well as the field survey results.
IV. Time Limits for Handling Procedures:
The authorizing party shall make a payment for the survey charges at any business office of the municipal Communications Bank by presenting the LETTER OF NOTIFICATION FOR PAYMENTS ON ENVIRONMENT MONITORING & SURVEYING, and fax the receipt of the said payment to the field survey office of the above Station. And the check and acceptance procedures shall be completed within 20 working days after receiving the payment receipt, under the precondition that such procedures are not to jeopardize the routine monitoring and surveying activities of this Station and not to affect the fulfillment of the missions and duties assigned by the superior authorities for monitoring and surveying.
V. The charge rates shall follow the YJH [1996] No. 64 document of A FORMAL REPLY ON MODIFICATION OF CHARGE RATES CONCERNING ENVIRONMENTAL MONITORING & SURVEYING issued by Guangdong Provincial Pricing Bureau.
