II. Procedures for Recording & Filing:

(I) The organization or individual subject to such control programs supplies the needed information documents as required;
(II) The information documents are to be verified on the spot for recording and filing;
(III) Receipts are delivered on the spot to the organization that meets the required conditions for recording and filing;
(IV) A re-application will be involved if the program (plan) is to be changed or altered.
III. Information Materials Needed for Recording & Filing Procedures:

(I) Program (plan) for implementing controls within specified time limit;
(II) Xeroxed copy of the valid certificate for the designing institution
